DentAssure, Our In-House Discount Dental Plan

At Millennium Way Dental Studio, we try to find ways to help our patients without dental insurance. We recognize that quality care is presently beyond the reach of millions of people and families each year. To offset the costs of care, we have developed an In-House Discount Dental Plan, so everyone can have state of the art dental care, at an affordable cost.

Our In-House Dental Plan is not an insurance, but rather a discount plan, good for treatment completed only at Millennium Way Dental Studio. The plan does not apply to any completed treatment, treatment in progress or any other promotions.

YEARLY MEMBERSHIP DUES

  • First Family Member $300
  • Each Additional Adult Member $275
  • Children (Age 1-14) $200
  • Periodontal Maintenance Member $450

(PLEASE NOTE:  ALL FAMILY MEMBERS MUST LIVE IN THE SAME HOUSEHOLD)

The Adult (15 and Over) Membership includes:

  • Periodic Exams (2 per year)
  • Routine Cleanings (2 per year)
  • Check-up X-Rays (PRN)
  • Fluoride Treatment (1 per year)
  • 15% off all other dental procedures*

The Children’s (Ages 1-14) Membership includes:

  • Periodic Exams (2 per year)
  • Routine Cleanings (2 per year)
  • Check-up X-Rays (PRN)
  • Fluoride Treatment (2 per year)
  • 15% off all other dental procedures*

Periodontal Maintenance Membership includes:

  • Periodic Exams (2 per year)
  • Periodontal Maintenance (3 per year- 4 month re-care appointments)
  • Check-up X-rays (PRN)
  • Fluoride Treatment (1 per year)
  • Scaling and Root Planing is billed separately
  • 15% off all other dental procedures*

  • This is your plan and it follows you regardless of who you or your spouse works for during the year.

Terms and Limitations of the Plan

  • This is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance.
  • It is good only for Millennium Way Dental Studio. Therefore, if you are referred to a specialist, they will NOT offer this discount.
  • Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, care, disability or workman’s comp type insurances are involved, this discounted plan can not be used.
  • This plan is NON-Transferable-Family members cannot be substitutes in for another family member.
  • It is NON-Refundable-no refunds given if patient chooses not to use their dental plan or the plan expires in 1 year.
  • Rates are subject to change annually.
  • CareCredit cannot be used to pay initial plan fee.
  • Payments for other services are due at time of service. If you choose to extend your payment for treatment by paying through CareCredit®, the discount is reduced by 5% due to merchant fees.
  • This offer cannot be combined with any other offers.
  • Dental services only, products are not included

There’s no ID card, no group or member number to bring! All your membership information will be kept in your electronic record. Your effective date is the day you sign up and your renewal date is the same date every year
