Your referrals mean the world to us! As a new, local, family owned business your referrals are what will drive our success. As our way of saying thank you for helping us grow this practice, we are initiating our “Share a Smile” Referral Program.
Historically the best new patients come from wonderful existing patients, so we ask you to tell all of your friends and family about our office.
With each referral you’ll receive an entry into our Quarterly Giveaway for the following prize:

How do you enter?
Refer friends and family by:
- Giving them a Share a Smile card.
- Telling them when they call to schedule an appointment to mention they were referred by you. Don’t worry if they forget, we ask all new patients who referred them.
- You can also print this form and give to your friends and family members…we’ll be sure to take good care of them!
What qualifies as a referral?
Each time you refer a member of your family or a friend, and they complete their first cleaning, exam, and x-rays with us you will be entered into our quarterly drawing.
Each referral equals one entry! The winner will be announced on our Facebook page.
